Are you talking about the plate that goes on the back of the block before the flywheel is bolted on?  I just took mine off when I installed the QuickTime one.  Not sure what you mean by T5 block plate though.  I didn't think they were that transmission specific.

They can be bought new.  http://www.cjponyparts.com/ford-racing-transmission-separator-plate-manual-5-0l-1979-1995/p/M7007B/?gclid=CMT30PyDxs0CFZaEaQodLOIHig

Here's mine:

Yep that's it. I don't know what it's particularly called. The aod version clocks the starter different apparently and will not bought up to the t5 bell. Found that out yesterday.

For sale?

Sure, make an offer and you pay shipping.

But if I were you I'd buy one of the brand new ones from eBay.

Here is one for $54 shipped.  http://www.ebay.com/itm/Mustang-T-5-T5-Bellhousing-Block-Plate-Ford-V8-289-302-351-USA-Aluminum-/262490729069?hash=item3d1daaaa6d:g:wvQAAOSw-YBXGAcF&vxp=mtr

$55 with expedited shipping.  http://www.ebay.com/itm/Mustang-T-5-T5-Bellhousing-Separator-Block-Plate-Ford-V8-289-302-351-USA-New-/371643252240?hash=item5687a9fe10:g:u~UAAOSwPhdVA3YO&vxp=mtr
